Body Art
The purpose of the Mansfield Board of Health Regulation of Body Art Establishments is to promote the knowledge and practice of sanitation, personal hygiene, sterilization, universal precautions and aftercare requirements regarding body art for the purpose of preventing injury to the client and transmission of disease. These regulations can be found here.
According to these regulations, permits are issued through the Mansfield Health Department permits for the following:
- Body Art Establishment
- Body Art Practitioner
- Body Art Practitioner Apprentice
All Health Department Permits are processed through an online platform located here: https://permiteyes.us/mansfield/loginuser.php
All permit applications must be submitted via this platform. Paper applications are no longer accepted. New users must register first by using the link on the website above or by going directly to: https://permiteyes.us/mansfield/userregistration.php
The following documentation is required to be uploaded to the PermitEyes accounts for the Application to be processed:
Body Art Establishment:
- Informed Consent Form
- Blank Injury and/or Complication Reporting Form
- Blank Exposure Incident Form
- Disclosure Statement
- Infection Prevention and Control/Exposure Control Plan with All Practitioner’s Signatures (template can be found here)
- Drawing of the Floor Plan (Not Applicable for Renewals)
- Insurance Declaration
Body Art Practitioner
- First Aid and CPR Certifications for Tattoo and Piercing
- Preventing Disease Transmission and/or Blood Borne Pathogen Training
- Course work/training on human anatomy (body piercing only)
- Course work/training on skin diseases, disorders, and conditions for tattoo and piercing
- Hepatitis B vaccination status or Declination Notification Letter
- Evidence satisfactory to the Board of Health of at least two (2) years’ experience, prior experience, or a completed approved apprenticeship program. (Not applicable for renewals)
- Recent identification photograph (with head and shoulders, front view)
Body Art Apprentices
- First Aid and CPR Certifications for Tattoo and Piercing
- Preventing Disease Transmission and/or Blood Borne Pathogen Training
- Course work/training on human anatomy (body piercing only)
- Course work/training on skin diseases, disorders, and conditions for tattoo and piercing
- Hepatitis B vaccination status or Declination Notification Letter
- Signed and Notarized agreement with a permitted Body Art Practitioner
- Recent identification photograph (with head and shoulders, front view)
If you don't have regular access to a computer, an online permit station has been established in Town Hall on the first floor.
